Worked for me just two days ago. Some credit card companies can/will

Actually, the poker sites will pay the insta-cash fees. Just deposit
with Neteller and use the insta-cash option. You have to furnish the
last 4 digits of your checking account to do this.

LOL. Most of *my* transactions are (small) withdrawals. That's when it tends
to get more difficult, particularly when you haven't deposited first.
